We hope you find this website easy to use. There are links to all pages on the left of the screen, these will lead you first to subsections and
then the final page. Many pages have additional links to other websites we think
could be useful, these are blue and underlined.
Advice for Internet Request Forms
To fill out request forms, click on the relevant page for example booking an appointment or changing your address. Here you will be asked for your first name, surname, date of birth and phone number as well as your email address if you can provide this. You can then press the button
in the middle of the screen to move on to fill out your request.
If you would like confirmation we have received your details, tick the box at
the top of the first screen. For further advice see the guidance at the top of
each request screen.
